For the nightmare staff go to the quest "The Prime Evil" on nightmare and purchase the Plan from the Quartermaster next to the healer for 1 gold.For the hell and inferno staff just the same.To upgrade you only need the staff of the difficulty before and an amount of gold (200k for nightmare, 500k for hell and 1M for inferno I believe)
