I will try to make an small compilation:
Once Indy has the request for balancing and what is necessary, they create the items and test them, when they think all will be ok then they send the patch to Acclaim who test it in their test server, checking the corrected things, the bugs, etc.
Then a full report is sent to Indy who works in fixing all.
This process is repeated until all is working correctly, then the patch is released. And then, in the live server the items may have a different behavior or a new bug may come.
Note that I have omitted the translation time that it is very important when the patch includes more than few new items.
